Creating Space for Better Breathing

At our practice, clear aligner therapy (such as Invisalign or similar systems) is a versatile tool in our airway-centered approach. We go beyond straightening teeth to widen dental arches, increase tongue space, and support overall craniofacial function. This is particularly valuable for both growing patients and adults seeking less invasive options for breathing and sleep improvements.

Widening the Arch and Creating More Tongue Space with Clear Aligners

Traditional orthodontics often focuses on alignment within existing space. In our airway-focused protocol, we use digitally planned clear aligners to achieve controlled arch expansion (primarily dental and alveolar widening).

  • How It Works: Precision programming allows gradual widening of the upper and/or lower arches. This increases the transverse dimension without the bulk of traditional expanders in many cases (or in combination with them).
  • Benefits for Tongue Space: A wider arch creates more room inside the mouth. Crowded or narrow arches often force the tongue to sit low and/or back in the airway due to reduced oral volume. Broadening of the dental arch provides the necessary space for the tongue to rest properly against the palate (roof of the mouth) in its natural, elevated position.

The Airway Connection:

  • Proper tongue posture acts like a natural "scaffold," helping maintain an open airway.
  • With more space, the tongue is less likely to fall backward (especially during sleep), reducing obstruction in the oropharyngeal airway.
  • This supports nasal breathing, improves airflow, and can contribute to better sleep quality and reduced snoring.

Patients often report easier breathing, less mouth breathing, and improved energy as these changes take effect.

Evidence of OSA Improvement with Clear Aligner Therapy

Research supports the potential of clear aligners to positively influence airway dimensions, particularly through arch expansion and improved tongue posture:

  • Studies using CBCT imaging have shown changes in tongue position relative to the palate and incisors in a majority of cases after aligner treatment (e.g., around 62% in one retrospective analysis), with measurable improvements in pharyngeal airway space for patients with constricted airways.
  • Arch expansion with clear aligners can increase nasal airway volume and intermolar distance, with notable benefits in pediatric patients and positive trends in adults.
  • When combined with mandibular advancement or used in comprehensive plans, aligners have demonstrated improvements in upper airway morphology, hyoid bone position, and OSA symptoms in case reports and reviews. One example involved combining aligners with a specialized sleep appliance, leading to better sleep and breathing outcomes.

While clear aligners alone are often not a cure for moderate-to-severe OSA they can be a valuable part of a multidisciplinary strategy.  The best results have been seen in patients with mild to moderate OSA. Results vary by individual anatomy, severity, and adherence, but many patients experience meaningful symptom relief through enhanced tongue space and airway support.
