MARPE / MASPE

MASPE/MARPE (Mini Assisted Rapid or Slow Palatal Expansion)

MASPE stands for Miniscrew-Assisted Slow Palatal Expansion. It's an advanced orthodontic treatment used primarily in adults and older teens to widen the upper jaw (maxilla) when it's narrow.

This technique is a variation or gentler alternative to the more commonly known MARPE (Miniscrew-Assisted Rapid Palatal Expansion).

DOMEzero

Dr. Buxton provides a more specific method.  He utilizes the DOMEzero protocols for expansion in adults.

What is DOMEzero?

DOMEzero is an innovative, synchronized treatment protocol for adults that combines minimally invasive palatal expansion with clear aligner therapy. It addresses narrow palates, crowded teeth, and airway issues more comfortably and aesthetically than traditional methods.

Developed as an evolution of MARPE techniques, DOMEzero (often using the DomeZero appliance) allows for parallel expansion of the maxilla while simultaneously aligning teeth — avoiding the noticeable diastema (gap) between front teeth that many patients worry about.

Key Benefits for Adults

  • Improved nasal breathing and airway volume
  • Better sleep quality and reduced snoring/sleep apnea symptoms
  • Wider, more attractive smile
  • 100% Skeletal Expansion
  • Improved tongue posture and function
  • Reduced TMJ discomfort and facial tension
  • No large gaps between teeth during treatment

How DOMEzero Works

It uses a custom bone-anchored expander combined with clear aligners in a coordinated protocol. The expansion creates space, and aligners guide teeth into their ideal positions progressively.

Key Features of DOMEzero

  • Miniscrews (TADs — Temporary Anchorage Devices): Small titanium screws are placed into the palatal bone to anchor the expander directly to the bone (skeletal anchorage), rather than relying mainly on the teeth. This allows true skeletal expansion instead of just moving the teeth.
  • Slow expansion: Unlike rapid methods (where the expander is activated quickly, often 0.5–1 mm per day), DOMEzero uses gradual, gentle activation over weeks to months This promotes more natural bone remodeling and suture separation.
  • Non-surgical: No need for invasive surgery like traditional SARPE (Surgically Assisted Rapid Palatal Expansion).
  • No Diastema: The use of clear aligners in conjunction with palatal expansion removes on of the biggest detractors to pursuing treatment.  With DOMEzero you can avoid the large diastema and cut treatment time in half.
